Covering your care

Shine Integrative Physical Therapy is a direct-pay clinic meaning that we do not bill your insurance company. You will receive a Super Bill which you can submit to your insurance. We use Simple Practice as our electronic medical records and this system will automatically send you all Super Bills for the month prior on the first day of the next month. For example, on August 1st you will receive an email reminder that the PDFs for July’s Super Bills are available on your Patient Portal. You can then submit those Super Bills directly to your insurance. Your payment will be eligible for reimbursement or credit towards your deductible based on your out-of-network benefits with your insurance company. Lastly, the Simple Practice app will send you a receipt on the day of your visit but this receipt is not meant for insurance, just your records.

  • We strongly encourage you to you call your insurance company prior to your first visit to learn about your coverage with us. We are happy to answer any questions after you have spoken with your insurance to FILL OUT THIS INSURANCE QUESTIONS FORM.

    • If you learn that your insurance requires prior authorization, tell us immediately as this is a time-sensitive process. Most plans through United HealthCare, HealthNet, and Blue Cross Blue Shield will require this or you will not receive reimbursement nor credit for your payments here. We will work to obtain that for you for a small administrative fee.

    • You may consider using the services of Reimbursify to take care of submitting your Super Bill to your insurance. They charge a small fee per each claim (visit) and have an app to ease the process so that you can focus on healing!

    • For clients with Medicare insurance, please contact me prior to your first visit to discuss your coverage.

  • Cost of teen and adult sessions:

    • Evaluation 75-minutes for $300

    • 60-minute follow-up treatment for $240

    • 75-minute follow-up treatment for $300

    • Shorter visits will be pro-rated in 15-minute increments to coordinate with insurance rules

    • Telehealth visits: 30 minutes for $90 and 45 minutes for $135

  • Cost of sessions for infants and children (under 13 years-old) are the same rate, however:

    • Infants will generally have 45-minute sessions reserved but may require no more than 30 minutes of treatment. The pro-rated 15-minute insurance rule will guide billing. As a courtesy, up to 15 minutes of time to accommodate any feeding, diaper changes, or breaks will be complimentary.

    • Toddlers, pre-school and elementary school children will also generally have 45 minutes reserved. Their session may include up to 15 minutes of complimentary transitional time (reading, playing, breaks, etc), if needed.

  • Payments can be made via our secure Stripe or Square settings, Venmo, cash or check. We also accept HSA and FSA cards as well as check or cash. Here is an IRS publication covering how you can best use your Health Savings Accounts.

  • No refunds are available on used treatment supplies nor on custom-made medical equipment such as foot orthotics.
